Shift: 8hr Day Shift (40)

Shift Information: 8am

On-call, Call Back Requirements: None

Weekend Rotation: Every 4th weekend with a take back day during the week

Job Description

This position is 100% in the field, with nurse driving from one patient to the next. There is no "office" to report to, all meetings held virtually, and nursing supplies are delivered. Initial training required on site in Oakbrook for 2-3 days. Case Manager can expect to see 20-30 patients per week. Expectation is for 6 routine visits per day. Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc. Care consisting of education on medications, disease management and self-care along with performing skills such as wound care, infusion, line maintenance, ostomy care, etc and teaching patients how to perform care for these as well. EPIC/Computer charting. Sapphire IV pumps. Traveler will be provided Advocate laptop for charting. Any solid color scrubs acceptable. Candidate MUST provide their own transportation to/from all patient visits.

Candidate MUST HAVE

  • 1 year Home Health RN experience
  • Active BLS (AHA) and state RN license (IL)
  • Wound care experience (wound vacs, pleural drains and ostomy care).
  • Able to complete peripheral blood draws, central line care, and IV admin through multiple devices (elastomeric, IV pumps).
  • Excellent assessment skills.
  • Experience with computer documentation.
  • Reliable transportation, active driver's license, and insurance.
