-------------------------------------------------------------------------------What You’ll Do

As a Home Health RN, you’ll manage a dedicated caseload and provide exceptional care by:

• Conducting start-of-care, routine, and follow-up visits

• Completing thorough patient assessments in the home

• Creating, implementing, and updating individualized care plans

• Monitoring patient status and responding to changes in condition

• Collaborating with physicians, supervisors, and interdisciplinary teams

• Documenting visits accurately and timely in the EMR

• Following all state, federal, and agency standards of care

• Protecting patient privacy and maintaining HIPAA compliance

-------------------------------------------------------------------------------What We’re Looking For

• Current, unrestricted RN license

• Minimum of 1 year of clinical nursing experience

• Associate or Bachelor’s Degree in Nursing preferred

• Prior Home Health or Hospice experience preferred

• HomeCare HomeBase experience a plus

• Reliable transportation, valid driver’s license, and auto insurance

• Willingness to travel within the service area (approximately 50%)
